As part of some cases, it may be necessary for us to obtain your medical records from your GP, hospital and any other medical centre you have received treatment (such as a physiotherapist, dentist or osteopath). If this is the case, we will send you a form to be completed and signed. This form gives us your authority to legally request copies of your medical records. It is important that you return this form to us as quickly as possible in order to avoid any delays to your claim.
